Anterior cross-bite management in preadolescent child with Clarke’s appliance

Sasha came to clinic with his father complaining for cross-bite. We solved this problem with removable appliance


Both Sasha and his father noticed that lower incisors cover upper incisors much time before, but they were not visiting orthodontist until they were referred by pediatric dentist.


Mixed dentition, mesial occlusion 93 class malocclusion).

In similar cases we usually conduct 2-step treatment. First, with removable appliance or bite blocks we put incisors in normal position. Second, in permanent dentition, we move all teeth in correct position.

In this case bite blocks or removable appliance with bite plane could not be efficient, partial bracket system was rejected by patient. So I offered them some rare modification of Clarke’s appliance, so-called reverse Twin-Block.

It consists of 2 removable plates, one for upper jaw and one for lower.

It is supposed to be worn 24 hours a day, only being removed for teeth and appliance cleaning.

Sasha has been wearing the appliance very well, with periodic visits once in 2 months.

Treatment time and cost

Treatment took 9 months.

After this treatment we can see normal incisors overjet.

The next stage is supposed to be bracket system treatment. Cost of treatment 3600 AED (Twin-Block appliance).
