Carolina turned to the Polar Bear General Dental Clinic for the removal of her wisdom tooth on the lower right jaw. Other clinics refused to remove the tooth due to its anatomical peculiarities, fearing damage to the nerve during extraction and potential loss of sensation in the lips and chin on the right side.

Initial Situation:

On computer tomography and three-dimensional reconstruction of the jaw, we see that the inferior alveolar nerve runs between the roots of the wisdom tooth (marked by a red line), and this tooth is pressing against the adjacent molar tooth.

Since any extraction in our clinic is performed using atraumatic methods and magnification, we assured Carolina that the risk of sensory loss was minimal.

    Course of the Operation:

Under local anesthesia, access to the tooth was formed. Initially, the part of the tooth abutting the neighboring one was separated. Then, under magnification control, the wisdom tooth was carefully removed without damaging the inferior alveolar nerve, which could be visualized in the resulting socket (marked in white).

After the operation, we were able to see where the nerve ran between the roots of the removed tooth (indicated by an arrow).

On the follow-up computer tomography, we verified that the tooth was completely removed.

The next day, Carolina reported that sensation had fully returned after anesthesia and extraction. A week later, the patient informed us that they were experiencing no discomfort, and we removed the stitches.
