Treatment of dentin caries and aesthetic restoration of the tooth

Valeria came to the clinic for a routine check-up. The patient had her own CBCT scan, which revealed hidden caries on a tooth. Diagnosis: dentin caries on tooth 2.5.

Primary photographic protocol, where we can immediately see the presence of hidden caries. The patient doesn’t notice it, but the most dangerous part is that they don’t feel it! Hidden caries rarely exhibits symptoms until it progresses to pulpitis (inflammation of the tooth nerve).

Treatment is performed under the microscope. We replaced the filling on the adjacent tooth (it changed from white to pink) because we’ll be treating it soon too. There is a lot of work to be done there since the tooth is already non-vital. For now, we are saving the vital tooth from caries.

We isolate the working area with a rubber dam to prevent water from entering the oral cavity.

We remove some tissue and observe the carious process from both sides (contact surfaces).

This is one of the rare cases where the adjacent tooth surfaces are not affected (diagnosed in time).

Here, we changed the rubber dam to a denser one and completely removed all the caries. The tooth is prepared for subsequent restoration. There is pigmentation in the fissure of the tooth. This is not carious.

Special matrices are placed to create the lateral walls, and we check their tight fit to the tooth.

We restored the walls, supplemented the missing anatomy.

And we achieved a beautiful, natural restoration of the tooth, which is perfect in terms of the bite.
